- Vertigo of vestibular origin, including Meniere’s syndrome
- Labyrinthitis and other acute vestibular disturbances
- Nausea and vomiting of varied cause, including post-operative and drug-induced emesis
- Nausea and vomiting associated with migraine
Contraindications
- Known hypersensitivity to prochlorperazine or to other phenothiazines
- Comatose states or marked central nervous system depression, including depression caused by alcohol or other depressant drugs
- Parkinson’s disease and other extrapyramidal movement disorders
- Existing blood dyscrasias, bone marrow depression or phaeochromocytoma
- Infants and young children; caution in the elderly and in epilepsy, hepatic or renal impairment, narrow-angle glaucoma and prostatic enlargement
Side effects
- Extrapyramidal reactions, including acute dystonia with spasm of the face, neck, jaw or eye muscles, reported most often in younger patients and early in treatment – urgent medical attention is needed
- Akathisia, felt as inner restlessness and an inability to sit still
- Parkinsonian features such as tremor, rigidity and slowed movement, particularly in elderly patients
- Tardive dyskinesia, an involuntary movement disorder that can follow prolonged treatment
- Drowsiness, dizziness, dry mouth, blurred vision and postural low blood pressure
- Raised prolactin, presenting as breast symptoms or menstrual irregularity
- Rarely, neuroleptic malignant syndrome with fever, muscle rigidity and altered consciousness, which is a medical emergency

How is Stemmic MD different from cinnarizine tablets for vertigo?
They reach vertigo by different routes. Prochlorperazine in Stemmic MD is a phenothiazine that blocks dopamine D2 receptors at the chemoreceptor trigger zone and sedates the vestibular apparatus centrally. Cinnarizine brands such as Spynal 25 Tab and Spynal D Tab act on the labyrinth through calcium channel and antihistaminic effects, with domperidone added in the combination as a peripheral prokinetic.
Does Stemmic MD cause movement-related side effects?
Yes, this is the warning that matters most. As a dopamine antagonist, prochlorperazine can cause extrapyramidal reactions – acute muscle spasm of the face, neck or eyes, restlessness, tremor and rigidity, and tardive dyskinesia after prolonged use. Younger patients and the elderly are most at risk. Report any abnormal movement to your physician immediately.
