Indications
- Vertigo of vestibular origin accompanied by nausea and vomiting
- Meniere’s disease where emesis is a prominent feature of the attack
- Vestibular neuronitis and labyrinthitis with associated vomiting
- Motion sickness in which nausea and vomiting dominate the symptom picture
Contraindications
- Known hypersensitivity to cinnarizine, to domperidone or to other piperazine derivatives
- Prolactin-releasing pituitary tumour (prolactinoma)
- Gastrointestinal haemorrhage, mechanical obstruction or perforation, where increased gut motility would be hazardous
- Moderate to severe hepatic impairment
- Known QT-interval prolongation, significant electrolyte disturbance or underlying cardiac disease, and concurrent use of QT-prolonging medicines or potent CYP3A4 inhibitors
- Parkinson’s disease and other extrapyramidal movement disorders
Side effects
- Drowsiness, sedation and dry mouth
- Gastrointestinal discomfort
- Raised prolactin, which may present as breast tenderness, galactorrhoea, gynaecomastia or menstrual irregularity
- Headache
- Extrapyramidal reactions such as tremor or stiffness, more likely in elderly patients and with prolonged use
- Rarely, disturbance of cardiac rhythm and QT-interval prolongation

What is Spynal D Tab used for?

Why does Spynal D Tab contain Cinnarizine 20 mg when Spynal 25 Tab has 25 mg?
Spynal D is a fixed-dose combination, and its cinnarizine content is 20 mg rather than the 25 mg used in the single-agent Spynal 25 Tab, with Domperidone 15 mg added to control vomiting. The two brands are therefore not equivalent tablet for tablet, and the choice between them is made by the prescriber on the symptom pattern.
Who should not take Spynal D Tab?
It is avoided in people sensitive to cinnarizine or domperidone, in prolactinoma, in gastrointestinal bleeding, obstruction or perforation, in moderate to severe liver impairment, in Parkinson's disease, and where there is QT-interval prolongation, an electrolyte disturbance or cardiac disease, or treatment with QT-prolonging medicines.
