Indications
- Gastro-oesophageal reflux disease where heartburn is accompanied by regurgitation, belching or post-prandial fullness
- Functional (non-ulcer) dyspepsia with delayed gastric emptying
- Acid-peptic disease presenting with nausea and vomiting
- Gastritis and duodenal ulcer symptoms in which a prokinetic is needed alongside acid suppression
- Reflux in patients on several long-term medicines, where a proton pump inhibitor with limited cytochrome P450 interference is preferred
Contraindications
- Hypersensitivity to pantoprazole, to domperidone, or to any substituted benzimidazole proton pump inhibitor
- Known QT-interval prolongation, significant electrolyte disturbance, or underlying cardiac disease including congestive heart failure – domperidone prolongs the QT interval and 30 mg per capsule is a substantial exposure
- Concurrent QT-prolonging drugs, or potent CYP3A4 inhibitors such as ketoconazole and erythromycin, which raise domperidone blood levels
- Moderate or severe hepatic impairment
- Situations in which stimulating gastric motility is unsafe: gastrointestinal haemorrhage, mechanical obstruction or perforation
- Prolactin-releasing pituitary tumour (prolactinoma)
Side effects
- Headache, dry mouth, diarrhoea or constipation, and abdominal cramping
- Raised prolactin from the domperidone component, causing breast tenderness, galactorrhoea or menstrual irregularity
- QT prolongation and, rarely, serious ventricular arrhythmia – palpitations, fainting or unexplained dizziness should be reported to a physician without delay
- With extended proton pump inhibitor therapy: low magnesium, reduced vitamin B12 absorption and increased fracture risk
- Uncommon: rash, urticaria and extrapyramidal movement effects

What does DSR mean in Panzomist DSR capsule?
DSR refers to the domperidone sustained-release design of the capsule. The domperidone is formulated to be released gradually rather than all at once, while the pantoprazole is enteric-coated so it survives stomach acid and is absorbed in the small intestine. The capsule shell simply carries both components together.
What is the difference between Panzomist DSR and a plain pantoprazole tablet?
A plain pantoprazole tablet only reduces acid production. Panzomist DSR adds domperidone, a prokinetic that speeds gastric emptying and reduces nausea. It is chosen when the patient's complaint is not heartburn alone but heartburn together with bloating, belching, early fullness or nausea. Because it contains domperidone it also carries cardiac cautions that a pantoprazole-only tablet does not.
Who should not take Panzomist DSR?
It should be avoided by anyone with a known QT-interval abnormality, an existing cardiac rhythm or heart failure problem, or a significant electrolyte disturbance, and by anyone with moderate to severe liver impairment. It is also unsuitable where increasing gut motility would be dangerous, such as gastrointestinal bleeding, obstruction or perforation. Tell your physician about all other medicines, particularly antifungals, macrolide antibiotics and anything known to affect heart rhythm.
