Indications

  • Essential hypertension not adequately controlled on telmisartan or amlodipine alone
  • Initial therapy in adults expected to need two agents to reach their blood pressure target
  • Substitution therapy for patients already stabilised on telmisartan and amlodipine taken as separate tablets
  • Hypertension where a single-pill regimen is preferred to support adherence

Contraindications

  • Pregnancy, and women planning pregnancy
  • Hypersensitivity to telmisartan, amlodipine or other dihydropyridines
  • Severe hypotension, cardiogenic shock, severe aortic stenosis and haemodynamically unstable heart failure after myocardial infarction
  • Biliary obstruction, severe hepatic impairment, bilateral renal artery stenosis, and concomitant aliskiren in diabetes

Side effects

  • Ankle and pedal oedema, typically reported less often than with amlodipine alone
  • Headache, flushing and palpitations
  • Dizziness and postural hypotension
  • Raised serum potassium and rises in serum creatinine
  • Fatigue and, uncommonly, gum swelling

What is the difference between Telmamist 40 and Telmamist-AM?

Telmamist 40 is single-agent telmisartan 40 mg. Telmamist-AM adds amlodipine 5 mg to the same telmisartan base, so it lowers blood pressure through two mechanisms – angiotensin receptor blockade plus calcium channel blockade – and is used when one agent has not brought readings to target.

Does Telmamist-AM cause ankle swelling?

Ankle and pedal oedema is the best-known effect of amlodipine, caused by arteriolar dilatation raising pressure in the capillaries. Combining it with an angiotensin receptor blocker such as telmisartan relaxes the venous side as well, and oedema is reported less often with the combination than with amlodipine alone. Report persistent swelling to your physician.

What is the composition of Telmamist-AM tablet?

Each Telmamist-AM tablet contains telmisartan 40 mg and amlodipine 5 mg.
