Indications
- Essential hypertension in adults not controlled on the two-drug combination of telmisartan and amlodipine
- Substitution therapy for patients already stabilised on telmisartan, amlodipine and hydrochlorothiazide as separate tablets
- Salt-sensitive or volume-dependent hypertension where a thiazide diuretic adds benefit
- Hypertension requiring three complementary mechanisms in a single daily tablet
Contraindications
- Pregnancy and breastfeeding
- Hypersensitivity to any component, or to sulfonamide-derived medicines
- Anuria, severe renal impairment, severe hepatic impairment or cholestasis and biliary obstruction
- Refractory hypokalaemia, hyponatraemia or hypercalcaemia, symptomatic hyperuricaemia, severe aortic stenosis and cardiogenic shock
Side effects
- Dizziness and postural hypotension
- Peripheral oedema, flushing and headache
- Electrolyte disturbance – low potassium, low sodium, raised calcium
- Raised uric acid with gout flares, and rises in blood glucose and lipids
- Muscle cramps, increased urination and photosensitivity

How is Telmamist-AMH different from Telmamist-AM?
Telmamist-AM is dual therapy – telmisartan plus amlodipine. Telmamist-AMH adds hydrochlorothiazide 12.5 mg, a thiazide diuretic that reduces sodium reabsorption and plasma volume. It is intended for patients whose blood pressure stays above target on the dual combination.
What tests are needed while taking Telmamist-AMH?
Because it contains a thiazide diuretic, physicians usually check serum electrolytes – particularly potassium and sodium – along with renal function, uric acid and blood glucose from time to time. Your physician will decide what monitoring you need and how often.
